Clinical RD II for Home Health.

Job Type: Full-time

Pay: $28.00 – $32.00 per hour

Work Location: Hybrid remote in Durham, NC 27560

Job Summary:

The Clinical Dietitian II works closely with the multidisciplinary healthcare team to coordinate evidence based medical nutrition therapy that is integrated and compatible with the patient focused medical goals.

Primary responsibilities include:

• This position is primarily non clinical. The RD will be monitoring patients and creating care plans with a multidisciplinary team for Home Health patients. 
• The ability to navigate multiple EMRs, will need to be comfortable with computers and tech savvy.  
• RD will be working from the corporate office.
  • Potentially a remote hybrid position – 3 days in office / 2 days remote from home.

Additional Responsibilities:

• Assists in achieving compliance with the Joint Commission, Centers for Medicare and Medicaid Services, Healthcare Facilities Accreditation Program (HFAP), or other accrediting agency standards to continuously improve the safety and quality of care to the public and to the organization’s performance improvement.
• Creates/updates evidence based protocols in area of specialty and trains others in order to ensure medical nutrition therapy practices are current and meet established guidelines.
• Participates in clinical research, on-the-job training, or QI projects as pertains to clinical specialty. Leads efforts to continually understand and measure quality of the specialty area practice in terms of structures, process, content, and outcomes. Designs and tests interventions to change process and systems of specialty practice area with the objective of improving quality outcomes. Serves on cross functional quality teams.

Credential Requirements:

• Bachelor’s degree from an ACEND accredited university
• Current CDR
• NC state license
