JOB OVERVIEW

• Works with Hospice Manager or Designee and prepares psychosocial evaluations
• Contract assignment: 13 weeks
• Location: Lacey, WA
• Start: 9/11/23
• Schedule:  Full time, 40 hours per week; 
• Setting: Home Health
• Est. weekly earnings: $1,850

MINIMUM JOB REQUIREMENTS

• Traveler is expected to have their own transportation.
• Two years social work experience or one year social work experience if in a health care setting.
• Current license to practice as a Master’s of Social Worker in the State of Michigan
• Valid driver’s license and proof of car insurance.
• Home care experience, Oasis exp. preferred
